Swing Bed/Case Management Services

Swing Bed Services

Swing Bed is a sub acute rehabilitation inpatient service that offers rehabilitation therapies 5-days a week to include: Physical, Occupational and Speech Therapy, to help our patient's transition to a long-term care facility or to rehabilitate from surgery, illness or accident. Additional skilled nursing services include medication administration and adjustment, IV therapy and wound care.

Whether you are admitted directly to NCCH or transferred from another hospital, you can depend on us to work with you, your physicians and healthcare team to deliver the care you need.

Services include:
Physical, Occupational and Speech Therapy are offered for patients that have had joint replacement or recent stroke, thereby improving their level of capability, independence and function
Respiratory Therapy
Continued wound care after recent surgery
IV therapy; chronic infection (antibiotics)
Pain control for terminal care
Waiting placement (either Assisted Living or Long-Term Care)
Patients needing detailed educational instruction such as insulin administration, testing and management; nutritional education
Social Services
Skilled Nursing
Coordinate Dental Services

Case Management

Case Management can read as follows: Case management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual's and family's comprehensive health needs through communication and available resources to promote quality cost-effective outcomes.
